Probing Pre-Operative Proficiency: Evaluating Resident Anesthetists' Knowledge of Patient Assessment and Risk Stratification in Himachal Pradesh

Abstract

Background: Anesthesiology plays a pivotal role in ensuring patient safety and optimal perioperative care. Resident Anesthetists in Himachal Pradesh shoulder the crucial responsibility of orchestrating the anesthesia process, beginning with pre-operative assessment. This study explores their knowledge of patient evaluation, history-taking and risk stratification, aiming to enhance their competence and improve healthcare standards. Materials and Methods: A cross-sectional survey was conducted among Junior and Senior Residents of Anesthesia in Himachal Pradesh. A sample of 100 participants was selected. A questionnaire assessed knowledge related to pre-operative assessment. Data were analyzed using Epi Info V7 Software. Results: The study revealed varying levels of knowledge among participants. Notably, 86% comprehended the purpose of pre-operative patient evaluation. Furthermore, 82% accurately described key components of a comprehensive assessment. However, 72% recognized the relevance of age, gender and lifestyle factors. While 82% understood risk stratification, 68% acknowledged the importance of interdisciplinary collaboration. Overall, 26 participants exhibited very good knowledge, 34 good knowledge, 21 fair knowledge and 19 poor knowledge of pre-operative assessment. Conclusion: This study highlights Resident Anesthetists' knowledge gaps in pre-operative assessment processes. While proficiency was evident in certain aspects, improvements are needed, particularly in recognizing the significance of lifestyle factors and interdisciplinary collaboration. Regular training and education programs are crucial to enhance their competence and, consequently, patient safety and healthcare quality in Himachal Pradesh.

INTRODUCTION

The realm of anesthesiology stands as a pivotal pillar in modern healthcare, ensuring patient safety and optimal perioperative care. Within this multifaceted domain, Resident Anesthetists shoulder the crucial responsibility of orchestrating the anesthesia process, a symphony of science and skill that begins long before a patient enters the operating theater. In the breathtaking landscapes of Himachal Pradesh, where the mighty Himalayas cradle the region, the healthcare system depends profoundly on the competence of Resident Anesthetists in delivering safe and effective anesthesia services.

 

The practice of anesthesia encompasses more than merely administering anesthetic agents during surgical procedures; it commences with an intricate process of pre-operative assessment. This pivotal phase involves a comprehensive evaluation of the patient, encompassing medical history, physical examination and meticulous risk stratification. Pre-operative assessment serves as the compass guiding anesthetists to make informed decisions, tailor anesthesia techniques and mitigate potential risks tailored to the individual patient's needs [1,2].

 

Patient evaluation, the cornerstone of pre-operative assessment, delves into the patient's medical history, current health status and any underlying conditions that may impact anesthesia management. It is a multidimensional inquiry aimed at uncovering medical intricacies, allergies, medications and previous surgeries, forming the basis for safe and personalized anesthesia planning [2,3].

 

Equally vital is the art of history-taking, where Resident Anesthetists adeptly navigate dialogues to unearth crucial information. They elicit details about the patient's medical history, family history, lifestyle habits and previous experiences with anesthesia. Such nuanced conversations facilitate a holistic understanding of the patient's physical and psychological health, essential for tailored anesthesia decisions [1,2].

 

Risk stratification, the ultimate outcome of pre-operative assessment, involves a methodical analysis of the patient's vulnerability to perioperative complications. Resident Anesthetists draw upon their knowledge and clinical acumen to categorize patients into risk groups, ensuring that anesthesia plans are aligned with the patient's unique needs and potential challenges. Competence in these pre-operative processes is integral to the Resident Anesthetist's role. Proficiency in patient assessment, history-taking and risk stratification safeguards against complications, enables informed anesthesia planning and enhances overall patient care [2,3].

 

This study embarks on a journey to comprehensively evaluate the levels of awareness and knowledge among Resident Anesthetists in Himachal Pradesh regarding pre-operative assessment processes. By scrutinizing their proficiency in patient evaluation, history-taking and risk stratification, we aim to shed light on areas that may benefit from further education and training.

 

The significance of this research lies in its potential to elevate the competency of Resident Anesthetists, ultimately contributing to the delivery of safer anesthesia services, optimized patient care and enhanced healthcare standards in the captivating terrain of Himachal Pradesh. As we delve into this exploration of pre-operative proficiency, we recognize the paramount role that this knowledge plays in ensuring patient safety and surgical success, all within the embrace of the Himalayas.

 

Objectives of the Study

The primary objective of this study is to gauge the levels of awareness and knowledge about Assess knowledge of pre-operative assessment process, including patient evaluation, history-taking and risk stratification among the Residents of Anesthesia in Himachal Pradesh.

MATERIALS AND METHODS
  • Research Approach: Descriptive

  • Research Design: Cross-sectional survey design

  • Study Area: Himachal Pradesh

  • Study Duration: Between April 2023 to June 2023

  • Study Population: The study's target population encompassed all Junior and Senior Residents of Anesthesia working in Medical Colleges in Himachal Pradesh

  • Sample Size: A robust sample size of 100 was determined using a 95% confidence level, an estimated knowledge level of 50% regarding Pre-Anesthetic Assessment, a precise 10% absolute error margin and a conservative 5% non-response rate

  • Study Tool: A google form questionnaire consisting of questions regarding socio-demography and knowledge regarding Pre-Anesthetic Assessment was created. The questionnaire was initially pre-tested on a small number of participants to identify any difficulty in understanding by the respondents

 

Description of Tool

 

  • Demographic Data Survey Instrument: The demographic form elicited information on participants’ background: age, gender, marital status, religion etc

  • Questionnaire: The questionnaire contains 20 structured knowledge related questions regarding Pre-Anesthetic Assessment. One mark was given for each correct answer and zero for incorrect answer. The maximum score was 20 and minimum score was zero. Scoring was done on the basis of marks as >80% (16-20) = very good, 60-79% (12-15) = Good, 41-59% (8-11) = Fair, <40% (<8) = poor

  • Validity of Tool: By the experts in this field

  • Data Collection: Data was collected under the guidance of supervisors. The google form questionnaire was circulated among all the Junior and Senior Residents of Anesthesia working in Medical Colleges in Himachal Pradesh for responses using online modes like e-mail and social media platforms like Whatsapp groups, Facebook, Instagram and Linkedin till the 100 responses were collected

  • Data Analysis: Data was collected and entered in Microsoft excel spread sheet, cleaned for errors and analyzed with Epi Info V7 Software with appropriate statistical test in terms of frequencies and percentage. 

  • Ethical Considerations: Participants confidentiality and anonymity was maintained

RESULTS

The primary objective of this study was to gauge the understanding of pre-operative assessment process, including patient evaluation, history-taking and risk stratification among the Residents of Anesthesia in Himachal Pradesh. A total of 100 participants took part in the study (Table 1).

In the present study 26 participants had very good knowledge (16-20 marks) towards Pre-Anesthetic Assessment, 34 had good knowledge (12-15 marks), 21 had fair knowledge (8-11 marks) and 19 having poor knowledge (<8 marks) (Table 2).

DISCUSSION

The central focus of this study was to assess the knowledge and awareness levels of Resident Anesthetists in Himachal Pradesh concerning pre-operative assessment, encompassing patient evaluation, history-taking and risk stratification. This critical phase in anesthesia management forms the bedrock upon which safe and personalized care is built, making it imperative for anesthetists to possess a deep understanding of these processes.

 

A substantial majority (86%) demonstrated a solid comprehension of the purpose of pre-operative patient evaluation in the anesthesia process. They understood that it forms the foundation for informed anesthesia planning, risk mitigation and tailored care. Similarly, an impressive 82% could accurately describe the key components of a comprehensive pre-operative assessment, reflecting their grasp of the multidimensional nature of this process. This foundational knowledge is essential for ensuring a systematic and thorough evaluation.

 

An overwhelming 86% were aware of the significance of assessing a patient's medical history and its role in identifying medical intricacies, allergies, medications and previous surgeries. This knowledge is pivotal in determining anesthesia safety and planning. Additionally, 79% could aptly identify factors considered when evaluating a patient's physical status and fitness for surgery, indicating their understanding of the importance of physical examination in assessing surgical risk.

 

Approximately 72% recognized the relevance of age, gender and lifestyle factors in patient evaluation, highlighting their awareness of the multifaceted nature of patient assessment. However, there is room for improvement in this aspect, as these factors can significantly impact anesthesia management. Furthermore, 78% appropriately identified specific medical conditions or comorbidities that should raise concerns during pre-operative assessment, showcasing their understanding of the role of comorbidity assessment in risk stratification.

 

An impressive 80% could outline pertinent questions when taking a detailed medical history from a patient, emphasizing the importance of comprehensive questioning. Similarly, 80% were knowledgeable about inquiring regarding medication use, allergies and previous surgeries during history-taking, essential for patient safety. However, there's room for improvement in recognizing the importance of asking about smoking and alcohol consumption habits.

 

An outstanding 82% demonstrated a sound understanding of risk stratification in the context of pre-operative assessment. They could aptly explain the American Society of Anesthesiologists (ASA) Physical Status Classification System and its significance. Additionally, 75% comprehended how to categorize patients into low, moderate, or high-risk groups based on their medical history and evaluation. This knowledge is pivotal in tailoring anesthesia plans to mitigate risks effectively.

 

Approximately 70% recognized potential complications associated with specific medical conditions and their impact on risk assessment, underlining their awareness of the intricate relationship between medical history and risk. Moreover, 68% acknowledged the role of interdisciplinary collaboration in risk stratification and patient management. However, there is room for further enhancement in this aspect to ensure comprehensive patient care.

 

The findings of this study align with prior research in the field, reflecting varying levels of knowledge among anesthesia practitioners in pre-operative assessment [1-3]. This underscores the importance of ongoing education and training programs for Resident Anesthetists to maintain and enhance their proficiency in this critical aspect of anesthesia management.

CONCLUSION

In conclusion, this study assessed the knowledge of Resident Anesthetists in Himachal Pradesh regarding pre-operative assessment processes, including patient evaluation, history-taking and risk stratification. While a significant proportion demonstrated commendable knowledge in various aspects of pre-operative assessment, there are specific areas where further training and education can yield improvements. These findings underscore the need for regular training programs and continuous medical education to enhance the proficiency of Resident Anesthetists, ultimately contributing to improved patient safety and healthcare quality in the region.
