Background: Eye donation and corneal transplants play a pivotal role in global eye health, addressing the pervasive challenges of visual impairment and blindness. While existing literature underscores the importance of awareness, regional nuances remain understudied. This research delves into the awareness levels in the culturally rich and geographically diverse landscape of Himachal Pradesh, aiming to contribute insights for targeted interventions. Materials and Methods: A cross-sectional survey involving 400 adults in Himachal Pradesh was conducted from August to November 2023. A Google Form questionnaire, covering socio-demography and 20 knowledge-related questions on eye donation and corneal transplants, was utilized. The study employed a descriptive research approach, with data analyzed using Epi Info V7 Software. A robust sample size was determined, ensuring a 95% confidence level and a 5% absolute error margin. Results: Of the participants, 70.5% demonstrated awareness of eye donation's medical advancements, while 30.75% understood the importance of corneal transplants. Notably, 36.5% acknowledged religious or cultural influences and 64.75% advocated for increased public education. Knowledge categories revealed 25.5% very good, 41.25% good, 20.25% fair and 13% poor understanding levels. Conclusion: This study unveils the layers of awareness and knowledge in Himachal Pradesh regarding eye donation and corneal transplants. Identifying cultural, religious and knowledge barriers provides a foundation for tailored interventions. The community's recognition of the need for enhanced education signifies an opportunity for impactful public health campaigns, fostering a collective responsibility toward eliminating avoidable blindness.

Amidst the serene landscapes, there exists an often unspoken realm of ocular health, where the potential for sight restoration through eye donation and corneal transplants holds profound significance. This study ventures into the heart of Himachal Pradesh to unravel the layers of awareness and knowledge surrounding eye donation and corneal transplants among its diverse populace.
The importance of eye donation and corneal transplants cannot be overstated in the broader context of global eye health. Visual impairment and blindness are pervasive challenges, with an estimated 2.2 billion people worldwide suffering from vision impairment or blindness, many of which are preventable or treatable through interventions such as corneal transplants [1,2]. The success of such interventions hinges not only on medical advancements but also on the collective awareness and willingness of communities to participate in the noble act of eye donation.
While the literature on eye donation and corneal transplants is substantial, the nuances of awareness at the regional level remain an understudied domain. The unique cultural tapestry, geographic isolation and societal intricacies of Himachal Pradesh necessitate a focused inquiry into the awareness levels of its inhabitants regarding these sight-restoring practices.
Recent research emphasizes the role of community awareness programs in enhancing eye donation rates and improving access to corneal transplants [3,4]. Understanding the specific challenges, beliefs and barriers within a population is crucial for tailoring effective public health campaigns [5,6]. Furthermore, the correlation between awareness and donation rates is underscored by studies demonstrating a positive impact on donation willingness when community members are well-informed about the benefits of eye donation [7,8].
As we embark on this exploration, the objectives of this study crystallize: to gauge the levels of awareness and knowledge about eye donation and corneal transplants among the general population of Himachal Pradesh. By unraveling the intricacies of awareness, identifying potential barriers and assessing the cultural and societal factors at play, this study aspires to lay the groundwork for targeted interventions that can potentially transform the visual landscape of this Himalayan region.
The significance of this research extends beyond the immediate goals of understanding awareness levels. It converges with the global efforts to eliminate avoidable blindness and reinforces the fundamental principle that the gift of sight, facilitated through eye donation and corneal transplants, is a collective responsibility and a beacon of hope for those in need.
Objectives of the Study
The objective of this study is to gauge the levels of awareness and knowledge about Eye Donation and Corneal Transplants among the general population of Himachal Pradesh.
Research Approach: Descriptive
Research Design: Cross-sectional survey design
Study Area: Himachal Pradesh
Study Duration: Between August 2023 to November 2023
Study Population: The study's target population encompassed all adults aged 18 and above who had been residents of Himachal Pradesh for a minimum of 12 months
Sample Size: A robust sample size of 400 adults was determined using a 95% confidence level, an estimated knowledge level of 50% regarding Eye Donation and Corneal Transplants, a precise 5% absolute error margin and a conservative 5% non-response rate
Study Tool:A google form questionnaire consisting of questions regarding socio-demography and knowledge regarding Eye Donation and Corneal Transplants was created. The questionnaire was initially pre-tested on a small number of participants to identify any difficulty in understanding by the respondents
Description of Tool
Demographic Data Survey Instrument: The demographic form elicited information on participants’ background: age, marital status, religion, employment, education and many more
Questionnaire: The questionnaire contains 20 structured knowledge related questions regarding Eye Donation and Corneal Transplants. One mark was given for each correct answer and zero for incorrect answer. The maximum score was 20 and minimum score was zero. Scoring was done on the basis of marks as >80% (16-20) = very good, 60-79% (12-15) = Good, 41-59% (8-11) = Fair, <40% (<8) = poor
Validity of Tool: By the experts in this field
Data Collection: Data was collected under the guidance of supervisors. The google form questionnaire was circulated among the residents of Himachal Pradesh for responses using online modes like e-mail and social media platforms like Whatsapp groups, Facebook, Instagram and Linkedin till the 400 responses were collected
Data Analysis: Data was collected and entered in Microsoft excel spread sheet, cleaned for errors and analyzed with Epi Info V7 Software with appropriate statistical test in terms of frequencies and percentage
Ethical Considerations: Participants confidentiality and anonymity was maintained
The primary objective of this study was to gauge the understanding of Eye Donation and Corneal Transplants among the general population of Himachal Pradesh. A total of 400 participants took part in the study, with 274 (68.50%) residing in rural areas and 126 (31.50%) in urban areas (Table 1).

In the present study 25.5% (102) participants had very good knowledge (16-20 marks) towards Eye Donation and Corneal Transplants, 41.25% (165) had good knowledge (12-15 marks), 20.25% (81) had fair knowledge (8-11 marks) and 13% (52) having poor knowledge (<8 marks) (Table 2).
The insights gathered from this study illuminate the intricate landscape of awareness and knowledge concerning eye donation and corneal transplants in the unique cultural milieu of Himachal Pradesh. With 400 participants contributing from both rural and urban domains, the robustness of our findings contributes to the understanding of regional perspectives on these critical aspects of ocular health.
The study reveals a commendable level of awareness (70.5%) regarding the significance of eye donation and its contribution to medical advancements. This aligns with studies emphasizing the role of awareness in fostering a positive attitude toward eye donation [3]. Nevertheless, the finding that 22.25% had considered becoming eye donors suggests potential for increased participation through targeted campaigns [8].
A significant knowledge gap is evident in the understanding of the importance of corneal transplants (30.75%). This finding underscores the need for tailored educational initiatives focused on the specific nuances of corneal transplantation, potentially addressing misconceptions or lack of information [6].
The study uncovers that 36.5% of participants acknowledge the influence of religious or cultural beliefs on perceptions of eye donation. This emphasizes the importance of culturally sensitive approaches in awareness campaigns, recognizing and addressing these beliefs to enhance community acceptance [5].
The identification of 6.5% acknowledging financial or logistical barriers to corneal transplants highlights an avenue for targeted interventions. Understanding and mitigating these challenges could contribute to increased access to transplantation services [4].
Notably, 47.5% recognize the role of communities and local healthcare providers in supporting individuals in need of corneal transplants. This emphasizes the potential impact of community-driven initiatives in enhancing the transplant ecosystem [9].
A resounding 64.75% believe there is a need for more public education and awareness campaigns. This aligns with global trends emphasizing the positive correlation between awareness levels and willingness to donate, emphasizing the importance of sustained educational efforts [7,8].
Comparative insights from global studies, such as those by Singh et al. [3] and Panda et al. [6], underscore the need for context-specific approaches in understanding and managing eye donation and corneal transplants. The study's focus on the cultural nuances of Himachal Pradesh contributes to the broader discourse on regional disparities.
The findings accentuate the critical role of tailored public health interventions in bridging knowledge gaps, dispelling myths and addressing cultural sensitivities. Strategies that engage communities, religious leaders and local influencers can enhance the effectiveness of awareness campaigns, thereby promoting a culture of eye donation and facilitating corneal transplants [10].
Limitations and Future Directions
Acknowledging limitations such as the self-reported nature of data and the online survey format, future research could delve into the specific cultural and religious beliefs influencing eye donation decisions. Comparative studies with other regions could further enrich our understanding of regional disparities and inform more targeted interventions.
In conclusion, this study serves as a pivotal step in unraveling the layers of awareness and knowledge regarding eye donation and corneal transplants in the heart of the Himalayas. The identified gaps provide actionable insights for crafting interventions that resonate with the unique cultural fabric of Himachal Pradesh, fostering a community-driven approach toward ocular health.
