The Impact of Cannabis on Perioperative Anesthesia and Postoperative Pain Management: A Mini Review

Abstract
Background: The number of people worldwide who use cannabis both medically and recreationally is steadily increasing. As such, health care providers must consider the health implications of cannabis for their patients. The perioperative and postoperative needs for patients who use cannabis may vary. However, little data exists examining this topic. As such, a mini-review was conducted to explore the literature on perioperative anesthesia and postoperative pain control for patients who use cannabis. Methods: A review of the literature from 2012-2022 was conducted by searching PubMed, CINAHL, Cochrane Review, Medline, and Google Scholar with a combination of the following keywords: marijuana, cannabis, anesthesia, postoperative pain management, and postoperative opioid use. After applying inclusion and exclusion criteria, removing duplicates, limiting to full-text articles, and focusing on articles relevant to the purpose of the review, a total of eight articles were selected. These articles were critiqued and data extracted by all authors. Three themes emerged from this mini-review: (1) perioperative anesthesia; (2) postoperative pain management; and (3) cannabis withdrawal. It was discovered that perioperative patients who use cannabis require increased amounts of anesthesia compared to non-cannabis users. These patients also reported higher pain levels postoperatively, necessitating increased analgesic agents. The concern for cannabis withdrawal was also identified for cannabis users and as such, literature supports that patients should use cannabis until the day of surgery and resume soon afterwards. Conclusions: The findings from this mini-review can help to add to the paucity of evidence guiding interventions related to the perioperative and postoperative management of patients who use cannabis. Future research endeavors is needed to shed further light on the effects cannabis has on the health of patients.
