Unveiling Proficiency in Post-Anesthesia Care: Assessing Awareness and Knowledge among Anesthesia Residents in Himachal Pradesh

Abstract

Background: Post-anesthesia care is a critical phase in perioperative management, profoundly impacting patient outcomes. This study assesses awareness and knowledge among Resident Anesthetists in Himachal Pradesh regarding extubation, recovery room monitoring, and pain management in the immediate post-operative period. Materials and Methods: A cross-sectional survey was conducted in Himachal Pradesh from April to June 2023. The study involved Junior and Senior Residents of Anesthesia in medical colleges. A sample of 100 participants was determined. A structured questionnaire assessed knowledge, scored as very good, good, fair, or poor. Data were analyzed using Epi Info V7. Results: The majority demonstrated good to very good knowledge levels across all domains, including extubation, monitoring, pain management, and communication. Criteria for discharge, PONV management, and documentation requirements were well understood. Areas for potential improvement included the use of regional anesthesia techniques and managing pediatric patients. Conclusion: Resident Anesthetists in Himachal Pradesh exhibit a strong foundation in post-anesthesia care, ensuring patient safety and quality care. Ongoing education and refinement of certain skills could further enhance their proficiency.

INTRODUCTION

Post-anesthesia care is a critical phase in the continuum of perioperative patient management. It is a period during which patients transition from the effects of anesthesia to a state of recovery and regain consciousness. The quality of care during this phase profoundly influences patient outcomes and satisfaction, making it an integral part of the perioperative process. This study ventures into the realm of post-anesthesia care, specifically focusing on the awareness and knowledge levels among Resident Anesthetists in the enchanting state of Himachal Pradesh.

 

The practice of anesthesia has evolved significantly over the years, driven by advancements in techniques, equipment, and the understanding of patient physiology. The role of Resident Anesthetists in this evolution cannot be overstated. These medical professionals serve as the primary caregivers during the perioperative period, ensuring patient safety, comfort, and a smooth recovery [1-2].

 

Extubation, monitoring in the recovery room, and pain management are pillars of post-anesthesia care, each demanding a unique skill set and a profound understanding of patient needs. Extubation is a delicate process involving the removal of the endotracheal tube, marking the transition from mechanical ventilation to spontaneous breathing. It requires precision and vigilance to prevent complications such as aspiration and airway obstruction .The recovery room, often referred to as the post-anesthesia care unit (PACU), is a specialized environment where patients are closely monitored as they emerge from anesthesia-induced unconsciousness. Monitoring encompasses various vital signs, including heart rate, blood pressure, respiratory rate, and oxygen saturation. Rapid identification and management of complications are essential for patient safety and well-being [2-3].

 

Effective pain management in the immediate post-operative period is vital for patient comfort and early mobilization. It involves the use of analgesic techniques and medications tailored to individual needs. Poor pain control can lead to increased morbidity, delayed recovery, and dissatisfaction among patients.

 

The proficiency of Resident Anesthetists in Himachal Pradesh in managing these aspects of post-anesthesia care is of paramount importance. Their knowledge and awareness not only ensure patient safety but also contribute to improved surgical outcomes and overall healthcare quality. This study embarks on a journey to assess the levels of awareness and knowledge among Resident Anesthetists in Himachal Pradesh regarding extubation, monitoring in the recovery room, and pain management in the immediate post-operative period. By doing so, we aim to shed light on their preparedness to navigate the complexities of post-anesthesia care, ultimately enhancing patient care and safety in this breathtaking region.

 

 

Objectives of the Study:

The primary objective of this study is to gauge the levels of awareness and knowledge about post-anesthesia care, including extubation, monitoring in the recovery room, and pain management in the immediate post-operative period among the Residents of Anesthesia in Himachal Pradesh.

MATERIALS AND METHODS
  • Research Approach: Descriptive

  • Research Design:  Cross-sectional survey design

  • Study Area: Himachal Pradesh

  • Study Duration: between April 2023 to June 2023

  • Study Population: The study's target population encompassed all Junior and Senior Residents of Anesthesia working in Medical Colleges in Himachal Pradesh

  • Sample Size: A robust sample size of 100 was determined using a 95% confidence level, an estimated knowledge level of 50% regarding Post-Anesthesia Care, a precise 10% absolute error margin, and a conservative 5% non-response rate

  • Study Tool: A google form questionnaire consisting of questions regarding socio-demography and knowledge regarding Post-Anesthesia Care was created. The questionnaire was initially pre-tested on a small number of participants to identify any difficulty in understanding by the respondents

  • Description of Tool: Demographic data survey instrument: The demographic form elicited information on participants’ background: age, gender, marital status, religion etc

  • Questionnaire: The questionnaire contains 20 structured knowledge related questions regarding Post-Anesthesia Care. One mark was given for each correct answer and zero for incorrect answer. The maximum score was 20 and minimum score was zero. Scoring was done on the basis of marks as>80%(16-20)=very good,60-79%(12-15) =Good,41-59% (8-11)=Fair,<40% (<8)=poor

  • Validity of Tool: by the experts in this field

  • Data Collection: Data was collected under the guidance of supervisors. The google form questionnaire was circulated among all the Junior and Senior Residents of Anesthesia working in Medical Colleges in Himachal Pradesh for responses using online modes like e-mail and social media platforms like Whatsapp groups, Facebook, Instagram and Linkedin till the 100 responses were collected

  • Data Analysis: Data was collected and entered in Microsoft excel spread sheet, cleaned for errors and analyzed with Epi Info V7 Software with appropriate statistical test in terms of frequencies and percentage

  • Ethical Considerations: Participants confidentiality and anonymity was maintained (Table 1)

RESULTS

The primary objective of this study was to gauge the understanding of post-anesthesia care, including extubation, monitoring in the recovery room, and pain management in the immediate post-operative period among the Residents of Anesthesia in Himachal Pradesh. A total of 100 participants took part in the study (Table 2).

DISCUSSION

The study aimed to assess the awareness and knowledge levels among Resident Anesthetists in Himachal Pradesh regarding post-anesthesia care, focusing on extubation, monitoring in the recovery room, and pain management in the immediate post-operative period. The findings provide valuable insights into their proficiency in managing this critical phase of perioperative care.

 

The study revealed that a substantial majority of participants correctly understood the primary goal of post-anesthesia care in the immediate post-operative period. Recognizing that this phase aims to ensure patient safety while transitioning from anesthesia to recovery is crucial. Participants demonstrated a good grasp of the process of extubation, including key criteria and considerations. This knowledge is essential for ensuring a smooth transition from mechanical ventilation to spontaneous breathing, minimizing the risk of complications [1-2].

 

Furthermore, participants exhibited awareness of the common complications or challenges that can occur during extubation and how to manage them. This competency in identifying and addressing potential airway issues contributes to patient safety. Understanding the importance of assessing the patient's airway and breathing immediately after extubation is a critical component of post-anesthesia care. The majority of participants recognized the significance of this assessment, ensuring early detection and intervention in case of airway or respiratory issues [1-3].

 

The study findings indicate that participants have a good understanding of the role of continuous monitoring in the recovery room and the parameters that should be monitored. Effective monitoring is vital for detecting and responding to post-operative complications promptly. The participants displayed a high level of knowledge regarding the criteria for discharge from the recovery room to the general ward or home. This knowledge is essential for ensuring the safe transition of patients to the next phase of care .Moreover, residents exhibited a strong grasp of how to recognize and manage post-operative nausea and vomiting (PONV) effectively. PONV is a common post-operative complication, and its early identification and management enhance patient comfort and satisfaction [2-3].

 

Pain assessment in the immediate post-operative period is a fundamental aspect of post-anesthesia care. The study participants demonstrated a good understanding of the principles of pain assessment, ensuring that patients' pain is adequately managed. Additionally, knowledge about multimodal analgesia techniques and their contribution to pain management reflects participants' awareness of contemporary pain management strategies. The participants exhibited awareness of the advantages and disadvantages of patient-controlled analgesia (PCA) systems. This knowledge empowers them to make informed decisions about pain management methods suitable for individual patients. Furthermore, understanding the potential complications and risks associated with opioids in post-anesthesia pain management is crucial in mitigating adverse effects and ensuring patient safety [1-2].

 

Residents showcased proficiency in managing pain in pediatric patients in the recovery room. The unique needs of pediatric patients require specialized care, and the participants' knowledge in this area is commendable. The study findings suggest that participants understand the role of regional anesthesia techniques in post-operative pain control. These techniques offer targeted pain relief and contribute to enhanced recovery. Residents displayed a good understanding of the assessment and management of respiratory depression in the post-anesthesia care unit (PACU). This knowledge is vital for preventing and addressing respiratory complications. Recognizing the significance of monitoring for surgical site infections and wound complications highlights participants' commitment to infection control and overall patient safety. The participants' understanding of how to effectively communicate post-operative care instructions to patients and their families ensures continuity of care and patient education [2-3].

 

Furthermore, knowledge about documentation requirements for post-anesthesia care, including monitoring records, reflects participants' commitment to maintaining accurate and comprehensive patient records. Understanding the guidelines for handover or transfer of care between anesthesia providers in the recovery room ensures a seamless transition of responsibilities, reducing the risk of errors and improving patient safety. The importance of ongoing education and training for residents in post-anesthesia care was recognized by the majority of participants. This reflects a dedication to continuous improvement and staying updated with the latest advancements in the field [1-3].

CONCLUSION

Resident Anesthetists in Himachal Pradesh exhibit a strong foundation in post-anesthesia care, ensuring patient safety and quality care. These results underscore the dedication of healthcare professionals in this region to provide safe and effective care during the critical post-operative period ongoing education and refinement of certain skills could further enhance their proficiency.
