Empowering Shimla: Deciphering Diabetes Awareness, Unraveling Risk Factors, and Advancing Preventive Strategies

Abstract

Diabetes Mellitus is a global health challenge with rising prevalence, and India, including Shimla, is particularly affected. Understanding community awareness of diabetes risk factors and preventive measures is crucial for informed interventions. Methods: A cross-sectional survey was conducted in Shimla, India, from April to July 2023. A structured questionnaire was used to assess awareness among 400 participants regarding diabetes risk factors and preventive measures. Data were analyzed using Epi Info V7 software. Results: Participants demonstrated good awareness levels, with 89.25% correctly defining Diabetes Mellitus, 81% identifying common symptoms, and 71% recognizing risk factors. Family history (67.75%) and obesity (62.25%) awareness was notable. Lifestyle (55.25%) and physical inactivity (63%) factors were acknowledged. Complications (44%) and glycemic index (25.25%) knowledge showed room for improvement. Exercise (65.75%) and healthy body weight (63.25%) benefits were recognized. Specific preventive measures (67.25%) were well-known, but awareness of smoking/alcohol impact (48.25%), screening guidelines (30.25%), and stress management (47.5%) was moderate. Complications (35.25%) and blood sugar control (57.25%) were acknowledged. Education's role (64.75%) in reducing prevalence was recognized. Conclusion: The study reveals commendable awareness of diabetes risk factors and preventive measures in Shimla's population. Targeted interventions are needed to improve knowledge in specific areas. Public health initiatives should focus on smoking/alcohol risks, glycemic index understanding, and screening guidelines. Enhancing understanding of complications and early intervention significance is essential.

INTRODUCTION

Diabetes mellitus, commonly referred to as diabetes, is a global health challenge characterized by chronic hyperglycemia, resulting from inadequate insulin production or utilization. The prevalence of diabetes has reached alarming levels worldwide, with an estimated 463 million adults affected in 2019, a number projected to rise to 700 million by 2045. This growing burden not only strains healthcare systems but also exerts a substantial economic impact, emphasizing the need for comprehensive strategies to address the issue [1-4].

 

In the context of India, often dubbed the "diabetes capital of the world," the problem is particularly acute. With an estimated 77 million adults afflicted by diabetes, the nation faces a significant public health crisis . Even in the serene and picturesque town of Shimla, nestled in the Himalayan region, the rising tide of diabetes has not spared its residents. The transition from traditional agrarian lifestyles to more urbanized and sedentary ones, coupled with a genetic predisposition, contributes to the increasing prevalence of diabetes in this region [5-8].

 

Understanding the level of awareness and knowledge within the community is crucial for designing effective awareness campaigns and targeted interventions. With this study, we seek to shed light on the prevailing levels of awareness regarding diabetes risk factors, including genetics, obesity, physical inactivity, and unhealthy dietary habits, among others. Additionally, we aim to explore the familiarity with preventive strategies such as regular physical activity, a balanced diet, and regular health check-ups [9-12].

 

Effective management of this epidemic necessitates a multifaceted approach, with a central focus on enhancing public awareness of diabetes risk factors and preventive measures. Accurate knowledge empowers individuals to make informed choices about their lifestyles, reducing the risk of developing diabetes and its complications   [13-16].   However,   there   is   a dearth  of comprehensive research assessing the public's awareness and knowledge of diabetes, specifically in the unique context of Shimla.

 

Addressing this knowledge gap is the primary objective of our study. Through a descriptive research approach and a cross-sectional survey, we aim to evaluate the awareness and knowledge regarding risk factors and preventive measures for Diabetes Mellitus among the general public of Shimla.

 

Objectives of the Study

To evaluate the awareness and knowledge regarding Risk factors and Preventive measures for Diabetes Mellitusamong general public of Shimla.

MATERIALS AND METHOD

Research Approach: Our study adopts a descriptive research approach to comprehensively assess the levels of knowledge regarding Risk factors and Preventive measures for Diabetes Mellitus among the general population of Shimla. The descriptive approach was chosen for its capacity to methodically collect and analyze data, enabling us to portray and comprehend the current awareness and understanding of the study participants concerning this specific subject matter.

 

Research Design

To obtain a holistic understanding of the awareness regarding diabetes risk factors and preventive measures in Shimla, we employ a cross-sectional survey design. This design facilitates data collection at a single point in time from a diverse cross-section of individuals. It ensures that we can capture a snapshot of the prevailing knowledge among the target population, providing valuable insights into the current state of awareness.

 

Study Area and Duration

Our research unfolds in Shimla, renowned for its cultural diversity and urban lifestyle. The study spans from April 2023 to July 2023, encompassing the phases of data collection and analysis. This duration is carefully selected to capture any potential seasonal variations or specific factors that might influence diabetes awareness.

 

Study Population and Sample Size

The study focuses on adults aged 18 years and above who have been residents of Shimla for a minimum of 12 months. To ensure robust findings, we determined a sample size of 400 participants. This sample size was calculated based on a conservative assumption of a 50% estimated knowledge level regarding diabetes risk factors and preventive measures, with a 5% absolute error margin, a 95% confidence level, and accounting for a 5% non-response rate. This rigorous approach ensures that our findings are representative and can be generalized to the larger population of Shimla.

 

Data Collection Tool

In our quest to assess diabetes awareness, we have developed a meticulously structured and validated Google Form questionnaire. The questionnaire comprises two sections: the first section captures socio-demographic information, including age, gender, marital status, religion, employment, and education. The second section comprises 20 thoughtfully designed questions aimed at evaluating participants' knowledge of diabetes risk factors and preventive measures.

 

Description of the Questionnaire

Our questionnaire is strategically designed to gauge participants' comprehension of diabetes. Each correct response is awarded one mark, while incorrect answers receive zero marks. Based on their scores, participants' knowledge levels are categorized into four distinct groups: very good (>80%), good (60-79%), fair (41-59%), and poor (<40%). This scoring system ensures a nuanced understanding of the overall awareness levels among the respondents.

 

Data Collection and Analysis

Data collection is meticulously executed under the supervision of experienced researchers. The Google Form questionnaire is disseminated through a variety of online platforms, including email, WhatsApp groups, Facebook, Instagram, and LinkedIn, ensuring that both rural and urban areas of Shimla are reached. The collected data is subsequently entered into a Microsoft Excel spreadsheet, where it undergoes rigorous validation for accuracy and completeness.

 

For data analysis, we employ the sophisticated Epi Info V7 software, utilizing appropriate statistical tests, such as frequencies and percentages, to present the findings in a clear and concise manner. This analytical approach allows us to draw meaningful insights from the collected data and provide a comprehensive overview of diabetes awareness among the residents of Shimla.

 

Ethical Considerations

Throughout the study, ethical principles are rigorously adhered to in order to safeguard the privacy and confidentiality of all participants. Informed consent is obtained from each respondent prior to their inclusion in the study. Personal information is treated with the utmost confidentiality, and stringent measures are in place to ensure that all data is anonymized, preserving the identities of the participants at all times.

RESULTS

The objective of this study was to evaluate the understanding of risk factors and preventive measures for Diabetes Mellitus among the general population of Shimla using a non-experimental descriptive survey. A total of 400 respondents took part in the study, with 259 (64.75%) residing in urban areas and 141 (35.25%) residing in rural areas.

In the present study 32.75% (131)  participants had very good knowledge (16-20 marks) towards Risk factors and Preventive measures for Diabetes Mellitus, 33.75% (135) had good knowledge (12-15 marks), 22.7.5% (91) had fair knowledge  (8-11 marks) and 10.75% (43) having poor knowledge  (<8 marks).

DISCUSSION

Diabetes Mellitus is a global public health concern that necessitates a comprehensive understanding of its risk factors and preventive measures. This study aimed to assess the knowledge of these crucial aspects among the general population of Shimla, India. The findings present a valuable insight into the current awareness levels regarding Diabetes Mellitus and serve as a foundation for public health interventions.

        

The study revealed that a substantial majority (89.25%) of participants correctly defined Diabetes Mellitus. This indicates a strong grasp of the basic concept of the condition, which is fundamental for early recognition and effective management.

        

Approximately 81% of the respondents could name common symptoms of Diabetes Mellitus. This knowledge is vital as it can prompt individuals experiencing these symptoms to seek medical advice and early diagnosis.Regarding risk factors associated with Type 2 Diabetes Mellitus, 71% of participants demonstrated awareness. Understanding these risk factors can empower individuals to make informed lifestyle choices and adopt preventive measures.

 

A significant proportion (67.75% and 62.25%, respectively) recognized the influence of family history and obesity on the risk of developing Diabetes Mellitus. Acknowledging these factors can encourage individuals to focus on preventive strategies. Over half of the participants (55.25% and 63%, respectively) were aware of the role of lifestyle factors and physical inactivity in Diabetes Mellitus development. This awareness underscores the importance of lifestyle modifications in prevention.

        

While 44% were aware of the long-term complications of uncontrolled Diabetes Mellitus, this figure indicates a need for more comprehensive education about the potential health consequences associated with the condition. A substantial majority (67.25% and 25.25%, respectively) recognized the importance of a balanced diet and the concept of glycemic index in Diabetes Mellitus prevention. This demonstrates an understanding of dietary factors in managing blood sugar levels.

       

 More than 60% recognized the benefits of regular exercise and maintaining a healthy body weight in preventing Diabetes Mellitus. These findings emphasize the significance of physical activity and weight management in risk reduction. 67.25% of respondents could name specific preventive measures for Type 2 Diabetes Mellitus. This indicates a relatively high level of awareness regarding actionable steps for prevention.

 

While 48.25% recognized the influence of smoking and excessive alcohol consumption on Diabetes Mellitus risk, and 66.5% acknowledged the importance of regular health check-ups for early detection, there is still room for improvement in these areas. Knowledge regarding recommended screening guidelines (30.25%) and the role of stress management (47.5%) in Diabetes Mellitus prevention appears to be areas where additional education may be beneficial.

       

 Around 35.25% recognized complications affecting the eyes, kidneys, and nerves, while 57.25% identified measures to control blood sugar levels. These findings suggest scope for enhancing awareness about complications and control strategies. Finally, 64.75% acknowledged the importance of education and awareness in reducing the prevalence of Diabetes Mellitus in the community. This highlights the role of public health campaigns and educational initiatives in combating the disease.

        

When comparing these findings with studies conducted in similar settings, the level of awareness regarding Diabetes Mellitus risk factors and preventive measures in Shimla's population appears relatively favorable [17-22]. However, specific areas that warrant further education and awareness campaigns include glycemic index understanding, screening guidelines, and the impact of smoking and alcohol consumption on risk.

CONCLUSION

In conclusion, this study provides valuable insights into the knowledge levels of risk factors and preventive measures for Diabetes Mellitus among the general population of Shimla. While there is a commendable level of awareness in many areas, targeted interventions are needed to address specific knowledge gaps. Public health initiatives should focus on raising awareness about the importance of regular health check-ups, glycemic index, and the risks associated with smoking and excessive alcohol consumption. Additionally, efforts should be made to enhance understanding of diabetes-related complications and the significance of early intervention and management.
