We present a unique case of a 71-year-old woman who was operated on for her first lumbar vertebra compression fracture. Bilateral D11-L3 pedicle screw fixation was done in a surgery lasting three hours. Postoperatively, the patient developed anisocoria with left pupillary dilation. With no visual complaints, anisocoria resolved spontaneously in 48 hours. Only two such cases have been reported in the literature. Although rare, preoperative scrutiny of patients can help obviate many distressing ocular complications.</abstract></article-meta></front><body /><back /></article>
